Diode laser for Percutaneous Laser Disc Decompresion (PLDD)

The Diode laser is indicated for Percutaneous Laser Disc Decompresion (PLDD), for treatment of back pain resulting from herniated discs.
The treatment principle of PLDD is based on the concept of the intervertebral disk being a closed hydraulic system. This system consists of the nucleus pulposus, containing a large amount of water, surrounded by the inelastic annulus fibrosus. An increase in water content of the nucleus pulposus leads to a disproportional increase of intradiskal pressure. A reduction of intradiskal pressure causes the herniated disk material to recede toward the center of the disk, thus leading to reduction of nerve root compression and relief of radicular pain. In PLDD, this mechanism is exploited by application of laser energy to evaporate water in the nucleus pulposus.
Laser energy is delivered by a laser fiber through a hollow needle placed into the nucleus pulposus. The needle is placed into the intervertebral disk under local anesthesia. Apart from evaporation of water, the increase in temperature also causes protein denaturation and subsequent renaturation. This causes a structural change of the nucleus pulposus, limiting its capability to attract water andtherefore leading to a permanent reduction of intradiskal pressure.

Comparison to Pharmaceutical Relaxants
This form of therapy works via the entire nervous system and is indicated in cases of entire body muscle seizure as in a traumatic accident or shock;however, depressing the horse’s entire nervous system is contraindicated in addressing specific muscle problems. Drug therapy can relax a general muscle mass, but since it affects the contraction as well as the release processes of the muscles, it diminishes the horse’s overall strength and capability.Though necessary in cases following acute trauma, as a treatment modality in chronic situations, it is often ineffective.
Comparison to Electrical Stimulation
Electrical Stimulation devices work by adding contraction to a muscle that already cannot find release. This may not be the best way to release muscle spasms. These devices can be useful for low levels of pain relief, for neuromuscular diagnosis, and for exercising a muscle with deficient nerve impulse
(nerve damage).
Comparison to Thermotherapy & Heat
Applied to a wide area, heat application is not accurate and is carried away by the bloodstream. It has neither depth nor durability and cannot affect deep tissue. The best use for this therapy is to relax a general area which affects the nerve endings and may bring some temporary comfort to a horse.
Comparison to Cold Hydrotherapy & Cryotherapy
Because the body always tries to maintain a steady body temperature, icing an area will actually produce increased circulation after several minutes of initial vasoconstriction. After acute trauma, it is the treatment of choice to stop hemorrhaging, reduce pain and keep swelling to a minimum. After this initial physiological function, other modalities are more efficient and effective in treatment.
Comparison to Magnetic Therapy
Studies show that the magnetic field produced by even the strongest magnet cannot and does not penetrate more than a fraction of an inch below the skin layer, making them ineffectual against deep muscle problems.
Rest
Rest is extremely important after acute injury; however, there are times when it becomes counterproductive. This is because resting and immobilization allow a spasm to become permanent. Muscle strength diminishes significantly after two weeks of stall rest and joints become stiff from lack of movement.In addition, confinement can be mentally taxing on a horse, resulting in spasms due to tension and stress.
Equine Chiropractic Treatments
High Velocity Thrust (HVT) deals with subluxations of the skeletal structure and facilitating normal space between joints, therefore enhancing joint function. Much benefit in the way of relieving pressure on the nerves, can be derived from chiropractic adjustments, when the situation calls for it.Though the muscles may derive some relief when treated by this method, equine chiropractic cannot relieve muscle spasms or knotted fascia and is not the best treatment for such situations. In addition, HVT has proven to cause muscle and fascial tearing, which may cause soreness and inflammation in a horse. This is why the technique must be done by a fully trained and experienced practitioner and why several states, will only allow licensed veterinarians to perform Equine Chiropractic.
Muscles have two primary jobs; either to hold bones together (static) or move them (dynamic). If a vertebral or joint misalignment has existed for anylength of time, the muscles will have memorized an “incorrect” setting. After chiropractic adjustment, these muscles do not automatically “right”themselves. Unless the muscles involved are released so that “correct” muscle memory can be reestablished, chiropractic adjustment will usually not hold. Hypertonic muscles will simply pull the horse out of alignment again.

Diode Laser for otolaryngology

This a lightweight, portable, battery supported ,980-nm wavelength Diode laser with a fiber optic delivery system is being used widely the field of otolaryngology, as it is less expensive.

The laser transmits radiation via a flexible quartz fiber optic cable that can be used with a set of intra nasal, micro laryngeal hand pieces. These hand pieces allow for optimal laser focusing.

Light at its particular wavelength is absorbed by tissues relatively independent of tissue type. Tissue response at this wavelength is determined by the absorption of water as well as by absorption of endogenous components such as hemoglobin and melanin. As a result, the diode laser has a large (up to 10 mm) penetration depth in biologic tissue, which makes it ideal for photo coagulation. By contrast, the Co2 laser has a very shallow penetration depth and thus is best used for tissue vaporization.

This laser compares favorably with several of the other thermal lasers used in otolaryngology; its tissue-cutting and coagulation effect is comparable to that of other commonly used lasers.

The Diode laser’s flexible fiber delivery system is compatible with hollow instruments, allows for coaxial vision, and is ideally suited for intranasal, oral and micro laryngeal use.

The Diode laser is housed in a portable case and light weight so it can be easily transported between the OPD and the operating room. The length of set-up time is generally less than 5 minutes, and no warm-up period is necessary. It is battery supported in case of power failure.

Use of Diode Laser in ENT Surgery

ENT Surgeons use different types of lasers like Co2, KTP, Nd.YAG and Diode Laser in their practice.
Diode Laser
This a lightweight, portable, battery supported ,980-nm wavelength Diode laser with a fiber optic delivery system is being used widely the field of otolaryngology, as it is less expensive.
The laser transmits radiation via a flexible quartz fiber optic cable that can be used with a set of intra nasal, micro laryngeal hand pieces. These hand pieces allow for optimal laser focusing.
Light at its particular wavelength is absorbed by tissues relatively independent of tissue type. Tissue response at this wavelength is determined by the absorption of water as well as by absorption of endogenous components such as hemoglobin and melanin. As a result, the diode laser has a large (up to 10 mm) penetration depth in biologic tissue, which makes it ideal for photo coagulation. By contrast, the Co2 laser has a very shallow penetration depth and thus is best used for tissue vaporization.
This laser compares favorably with several of the other thermal lasers used in otolaryngology; its tissue-cutting and coagulation effect is comparable to that of other commonly used lasers.
The Diode laser’s flexible fiber delivery system is compatible with hollow instruments, allows for coaxial vision, and is ideally suited for intranasal, oral and micro laryngeal use.
The Diode laser is housed in a portable case and light weight so it can be easily transported between the OPD and the operating room. The length of set-up time is generally less than 5 minutes, and no warm-up period is necessary. It is battery supported in case of power failure.
Application techniques
1. It can be used in contact mode, in which the tip of the fiber is placed directly on the surface of the tissue. This mode is effective for both photocoagulation and vaporization of tissue, depending on the power density. At the low- or mid-power range, coagulation occurs; at a higher level, vaporization occurs.
2. The laser can be used in near-contact mode, in which the tip of the fiber remains several millimeters apart from the tissue. This mode is largely used for tissue photo coagulation.
3. The laser can be used for interstitial laser-induced thermotherapy, a minimally invasive method of treating both benign and malignant tumors. The laser is placed through puncture holes in the skin, and its energy is absorbed by local tissues. This hyperthermic state causes either immediate or delayed tissue necrosis via coagulation.
4. In the clinic most cases can be done after administering a topical anesthetic and a decongestant spray to the nasal cavity, but micro laryngeal surgery needs General anesthesia with special fleximetalic tube to prevent fire hazard.

Conclusion
• Its portability and functional diversity make it an attractive alternative to the other laser machines like Co2 and KTP.
• Most cases can be done under LA in the OPD set up
• Very cost effective if other departments like, Dental and Ophthalmology share the equipment
