During the last decade, the use of laser technology has become more common in operative gynaecology in Norway. The first modality to be introduced was the carbondioxide, CO2-laser, used together with the colposcope for the treatment of cervical intraepithelial dysplasia and condylomata. The same laser has also been employed through laparoscopic technique, with the aid of modern microvideofacilities. Finally, using the hysteroscopic technique, the Neodymium-Yttrium-Aluminium-Garnet, the Nd-YAG-laser and similar diode laser, has been introduced for treating intrauterine conditions. The use of laser in operative gynaecology makes it necessary to change the attitude towards indications for operation. The possibilities are challenging, and the effect on the length of postoperative convalescence is especially rewarding. The introduction of lasers in gynaecology could have far-reaching consequences for efficient use of hospital resources.

Abstract


AIM: To evaluate the long-term efficiency of diode laser transscleral cyclophotocoagulation (DL Tscpc) for refractory glaucoma.
METHODS: Retrospective analysis was made in 431 eyes with refractory glaucoma treated by DL Tscpc with lower energy power (1.5-2.5W) and more laser spots (26-34). Intraocular pressure (IOP), visualacuity, ocular symptoms and complications were observed after DLTscpc. Patients were followed up over 36 months.


RESULTS: Mean pre-operative IOP was (56.2±12.7)mmHg which was significantly different from the final follow-up mean IOP (17.3±8.8)mmHg. (P < 0.05). The success rate was 94.4% (68/72). Postoperative visual acuity kept no change in 399 eyes; elevated in 30 eyes and decreased in two eyes. The pain sense disappeared or remarkably relieved. The main complications included mild uveitis and hyphemae. Atrophy of eyeball occurred in two eyes.


CONCLUSION: It is concluded that DL TScpc is a simple, safe and effective therapy for eyes with refractory glaucoma through long-term observation.

Diode laser for Percutaneous Laser Disc Decompresion (PLDD)

The Diode laser is indicated for Percutaneous Laser Disc Decompresion (PLDD), for treatment of back pain resulting from herniated discs.
The treatment principle of PLDD is based on the concept of the intervertebral disk being a closed hydraulic system. This system consists of the nucleus pulposus, containing a large amount of water, surrounded by the inelastic annulus fibrosus. An increase in water content of the nucleus pulposus leads to a disproportional increase of intradiskal pressure. A reduction of intradiskal pressure causes the herniated disk material to recede toward the center of the disk, thus leading to reduction of nerve root compression and relief of radicular pain. In PLDD, this mechanism is exploited by application of laser energy to evaporate water in the nucleus pulposus.
Laser energy is delivered by a laser fiber through a hollow needle placed into the nucleus pulposus. The needle is placed into the intervertebral disk under local anesthesia. Apart from evaporation of water, the increase in temperature also causes protein denaturation and subsequent renaturation. This causes a structural change of the nucleus pulposus, limiting its capability to attract water andtherefore leading to a permanent reduction of intradiskal pressure.

Surgery Laser for ENT and Gynecology

The gigaa laser can be used as a precision cutting and coagulation tool for a variety of surgical applications in the fields of ENT and GYN.

Laser energy can be used to vaporize, ablate and cut soft tissue while providing lateral thermal injury capable of coagulating blood vessels. When using 1470nm energy, the mechanism of action is absorption by water in the soft tissue.
